Applying to Sacred Heart, step by step
Applications are accepted on a rolling basis. Follow the steps below; your application is processed once every item is uploaded to the School Admin portal.
Submit an inquiry
Please complete an inquiry form for all grades before submitting an application. Inquiry form
Apply online $125 fee
Complete the Sacred Heart Admissions Application and pay the non-refundable application fee (per family) online.
Upload your documents
Add these to the application portal:
- Birth certificate
- Baptismal certificate (if applicable)
- Most recent report card plus the prior two years
- Most recent standardized test scores
Send two forms
Complete the Sacred Heart Transcript Release Form and submit it to both Sacred Heart and your child’s current school. Then ask your child’s teacher (ELA or Math only) to complete the Sacred Heart Teacher Referral Form: PreK–Grade 1 or Grade 2–Middle School.
Schedule testing
Readiness Screening for Preschool and Kindergarten, or reading and math placement testing for Grades 1–6.
Before the first day
Submit the State of Maryland Health Inventory forms and current immunization record to our School Nurse. Nurse’s Corner
